COVID-19 immunisations for young people age 12-15
Following national guidance the NHS Derbyshire and Derby City immunisation team will be offering the COVID-19 vaccination to young people age 12-15 on Thursday 11th November 2021.
The NHS are conducting the whole process and will continue to use their e-consent as with other vaccine programmes. The COVID-19 vaccination is voluntary so please ensure you complete the consent forms if you wish your child to be vaccinated.
Children must be 12 on the day of vaccination. Please only complete the consent form if your child will be aged 12-15 on the day of the vaccination.
This vaccination will be free of charge and the highest priority is to make it easy for children to access one dose of the vaccine before the winter. The main purpose of the COVID-19 school age vaccination programme is to provide protection to the children who receive the vaccine and may help to reduce transmission of COVID-19 in the wider population.

Please be aware that due to storage requirements of the vaccine the team will need to know the number of vaccines needed in advance and therefore the consent system will close 24 hours prior to the session in school. If your school session is a Monday, the form will close the Friday before.
